By submitting the Client Intake Form, I hereby affirm that I understand that Spirit Mentoring, Energy Therapy, Energy Healing, Brennan Healing Science and Reiki are stress reduction, relaxation and healing techniques. I acknowledge that the treatments administered are only for the purpose of helping me to relax, relieve stress, and to reinforce the body’s natural healing ability.

I understand that practitioners do not diagnose conditions, nor do they prescribe substances or perform medical treatment, nor interfere with the treatment of licensed medical professionals. I understand that energy therapy and healing energy awareness psycho dynamic therapy complement medical treatment, and it is recommended that I see a licensed physician or licensed health care professional for any physical or psychological ailment I may have.

I also understand that long term imbalance in the body may require multiple treatments to bring the body systems back into balance. I understand that self-improvement requires commitment on my part. I acknowledge my commitment to my self-improvement process. I recognize that a treatment program must be followed to be truly effective, just as prescribed medication is only effective if taken as directed.

Except in the case of gross negligence or malpractice, I or my representative(s) agree to fully release and hold harmless Melina Fuhrmann from and against any and all claims or liability of whatsoever kind or nature arising out of or in connection with my session(s).
